I understand and agree to the following statements concerning Adventist Youth On a Mission (referred to as AYM), Adventist Youth On a Mission Southeast (referred to as AYMSE), any affiliate, partner or branch of AYM or AYMSE, any organization, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches:
A. Photography/Filming. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, reserve the right to photograph or film any and all attendees or people on the premises of an event organized by AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, whether participating wittingly or unwittingly in an event or any activity organized by AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches. Any and all photographs or videos taken by AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are the sole property of AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, and may be used in any way such as but not limited to promotional content, social media, websites, or in any other way or distribution method decided upon by AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, reserve the right to monetize any images and/or videos containing the likeness and sound of voice of any and all attendees or people on the premises of an event organized by AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, whether participating wittingly or unwittingly in an event or activity organized by AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, without giving any compensation of any kind to anyone. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, hold full and unrestricted rights to images and/or videos containing the likeness and sound of voice of any and all attendees or people on the premises of an event organized by AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, whether participating wittingly or unwittingly in an event or activity organized by AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches.
B. Health. All attendees of an event organized by AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, attend at their own risk and are solely responsible for their health. AYM, AYMSE, their affiliates, partners, and branches, including but not limited to churches, ministries and businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not liable or responsible for the spread, infection, contamination, transmission and/or contraction of any disease, including but not limited to COVID-19, among attendees. AYM, AYMSE, their affiliates, partners, and branches, including but not limited to churches, ministries and businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not liable or responsible if an attendee tests positive for COVID-19 as a result of attending the retreat. AYM, AYMSE, their affiliates, partners, and branches, including but not limited to churches, ministries and businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches are not responsible or liable if an attendee infected with a disease, including but not limited to COVID-19, attends a retreat or takes part in any activity organized by AYM, AYMSE, their affiliates, partners, and branches, including but not limited to churches, ministries and businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches.
C. Meals. All attendees of an event organized by AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, eat any provided meals at their own risk and are solely responsible for what they eat or drink. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not responsible or liable in any way for the meals and their preparation and service, such as but not limited to hygiene measures and ingredients. AYM, AYMSE, their affiliates, partners, and branches, including but not limited to churches, ministries and businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not responsible or liable for the food served at a retreat organized by AYM, AYMSE, their affiliates, partners, and branches, including but not limited to churches, ministries and businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches. AYM, AYMSE, their affiliates, partners, and branches, including but not limited to churches, ministries and businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not liable or responsible, in the case of food poisoning, allergic reactions, or any other adverse reaction the food served may cause.
D. Safety. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches are not responsible for the safety and well-being of any participant and are not liable in the event of death, injury, or any other mishap, such as but not limited to kidnap, assault or harassment of any kind. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not responsible or liable for minors in any way. Parents/guardians alone are responsible and liable for the safety and well-being of their minors. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches are not liable or responsible in the event of any form of harassment by any attendee or team member, such as but not limited to sexual, racial, physical, mental, verbal or non-verbal. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not responsible or liable for any form of assault such as but not limited to sexual, physical, racial, mental, verbal or non-verbal. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not responsible or liable for anything lost, stolen, or missing.
E. Outreach and other activities. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not responsible or liable for anything that may happen when going/traveling to/from outreach or any other activity, or during outreach or any other activity. Each attendee chooses to participate at their own risk. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not liable or responsible in the event of death, injury, or any other mishap, such as but not limited to kidnap, assault or harassment of any kind, when going/traveling to/from outreach or any other activity, or during outreach or any other activity. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not responsible or liable for minors going/traveling to outreach or any other activity, or during outreach or any other activity. Parents/guardians alone are responsible and liable for the safety and well-being of their minors going/traveling to/from outreach or any other activity, or during outreach or any other activity. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches are not liable or responsible in the event of any form of harassment from any attendee or team member, such as but not limited to sexual, racial, physical, mental, verbal or non-verbal, when attendee is going/traveling to/from outreach or any other activity, or during outreach or any other activity. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not responsible or liable for any form of assault such as but not limited to sexual, physical, racial, mental, verbal or non-verbal, when attendee is going/traveling to/from outreach or any other activity, or during outreach or any other activity. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not responsible or liable for anything lost, stolen, or missing, when attendee is going/traveling to/from outreach or any other activity, or during outreach or any other activity. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not responsible or liable with whom an attendee or a minor rides with to/from outreach or any other activity. All who carpool or share a ride with anyone do so at their own risk. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not responsible or liable for anything that may happen in transit or while traveling to/from outreach or any other activity. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not responsible or liable for anything lost, stolen, or missing from someone going/traveling to/from outreach or any other activity, or during outreach or any other activity. All attendees who participate in outreach or any other activity do so at their own risk. AYM, AYMSE, or their affiliates, partners, branches, including but not limited to churches, ministries or businesses, directly or indirectly associated with AYM, AYMSE or their affiliates, partners and branches, are not responsible or liable for anything that may happen during outreach or any other activity.
